Is there any way to make MUSHclient recognize that a key is being held? MiniWindow hotspots can recognize MouseDown/MouseUp, but I don't know if there's a similar way to do this for Macros.

No, there is no support for KeyUp and KeyDown actions. Effectively they are merged by the underlying libraries into a key press.
